We make a record of the medical care we provide and may receive such records from others. We use these records to provide or enable other health care providers to provide quality medical care, to obtain payment for services provided to you as allowed by your health plan, and to enable us to meet our professional and legal obligations to operate this medical practice properly.
We are required by law to maintain the privacy of protected health information (PHI), provide you with notice of our legal duties and privacy practices with respect to PHI, follow the terms of the Notice that is currently in effect, and notify you following a breach of unsecured protected health information.
Treatment. We use medical information about you to provide your chiropractic care. We may disclose medical information to our employees, associates, and other health care providers who are involved in providing the care you need. For example, we may share your medical information with other physicians, therapists, or specialists who will provide services that we do not provide, or with a laboratory that performs diagnostic tests.
Payment. We use and disclose medical information about you to obtain payment for the services we provide. For example, we may provide your health plan or insurance company with information about the care you received so they can process your claim.
Health Care Operations. We may use and disclose medical information about you to operate our practice. For example, we may use your information for quality assessment activities, employee training, licensing, and other administrative purposes.

Individuals Involved in Your Care. We may disclose health information to a family member, other relative, close personal friend, or any other person you identify who is involved in your care or payment for your care.
As Required By Law. We will disclose medical information when required to do so by federal, state, or local law.
Public Health Activities. We may disclose medical information for public health activities including prevention or control of disease, injury, or disability; reporting births and deaths; and reporting adverse reactions to medications or products.
Health Oversight Activities. We may disclose medical information to a health oversight agency for activities authorized by law such as audits, investigations, inspections, and licensure.
Judicial and Administrative Proceedings. We may disclose medical information in response to a court order, subpoena, discovery request, or other lawful process.
Law Enforcement. We may disclose medical information for law enforcement purposes as required by law, or in response to a valid subpoena or court order.

Coroners, Medical Examiners, and Funeral Directors. We may release medical information to a coroner, medical examiner, or funeral director as necessary for them to carry out their duties.
Other uses and disclosures of your medical information not covered by this Notice or applicable laws will be made only with your written authorization. You may revoke your authorization in writing at any time, except to the extent that we have already acted on your authorization.
Specially Protected Information. Certain categories of health information — including psychotherapy notes, HIV-related information, and substance-use-disorder treatment records — receive additional protection under federal and California law. We will not use or disclose information in those categories except as those laws permit or as you authorize in writing.
Right to Request Restrictions. You have the right to request restrictions on certain uses and disclosures of your health information. We are not required to agree to your request, except that we must agree to a restriction on disclosures to a health plan for services you paid for in full out of pocket.
Right to Receive Confidential Communications. You have the right to request that we communicate with you about medical matters in a certain way or at a certain location. For example, you may ask that we contact you only at your work address or via email.
Right to Inspect and Copy. You have the right to inspect and obtain a copy of your health information maintained by this practice, including in electronic format if maintained electronically. We may charge a reasonable, cost-based fee for copies, within the limits California law allows, and we will respond within the time California law requires. To make a request, please call us.
Right to Amend. You have the right to request that we amend your health information if you believe it is incorrect or incomplete. We may deny your request in certain circumstances.

Right to Be Notified of a Breach. You have the right to be notified if there is a breach of your unsecured protected health information. We will notify you in writing within 60 days of discovering a breach that may have compromised your information.
Right to Restrict Disclosures for Out-of-Pocket Services. If you pay for a service or health care item out of pocket in full, you have the right to request that we not disclose your protected health information related to that service to a health plan for the purpose of payment or health care operations. We are required to honor this request.
We will not sell your protected health information without your written authorization.
We reserve the right to change this Notice. Any revised Notice will apply to health information we already have about you as well as any information we receive in the future. A copy of the current Notice will be posted at our office and on our website.
